What is Healthier SG?

Healthier SG is a national initiative by the Ministry of Health (MOH) that aims to help all Singaporeans take steps towards better health and extended quality of life in the years to come. Preventive care is key to living long and well. Healthier SG empowers you to stay healthy, detect and manage health issues early for a healthier you.

Who is Healthier SG for?

Healthier SG is designed for all Singapore residents, regardless of age or existing health conditions. Whether you are in excellent health or managing chronic illnesses, the initiative provides a framework to improve your well-being. It also engages healthcare providers, including general practitioners (GPs) and polyclinics, as integral partners in this health transformation journey.

What is the purpose of Healthier SG?

The primary goal of Healthier SG is to shift Singapore’s healthcare model from being reactive to preventive. This means placing greater focus on keeping residents healthy, rather than only treating illnesses. The initiative offers structured plans tailored to individuals’ health profiles, promotes health screenings, vaccinations, and chronic disease management, and connects people to resources for healthy living.

When was Healthier SG launched?

Healthier SG was officially rolled out in mid-2023, with various phases of implementation. The initial stage focused on residents aged 60 and above, followed by younger age groups in subsequent phases.

Where is Healthier SG available?

Healthier SG is accessible through participating healthcare providers across Singapore. Residents can enroll by selecting a family doctor from a network of approved clinics, which include both public polyclinics and private GPs. Additionally, resources and support programs are made available through community centers, fitness facilities, and digital platforms.

Why was Healthier SG introduced?

The initiative was introduced to address the challenges of Singapore’s aging population and rising prevalence of chronic diseases like diabetes, hypertension, and high cholesterol. By promoting preventive care and healthier lifestyles, Healthier SG aims to reduce the burden on healthcare resources while improving the overall quality of life for residents.

How does Healthier SG work?

Healthier SG operates through a collaborative approach involving residents, family doctors, and the community. Here’s how the program works:

  1. Enrollment: Residents enroll in the program by choosing a family doctor through the HealthHub app or website.
  2. Health Plans: Upon enrollment, the chosen doctor creates a personalized health plan based on the resident’s health profile, which may include dietary advice, exercise recommendations, and regular check-ups.
  3. Screenings and Vaccinations: Residents are encouraged to undergo free screenings and vaccinations.
  4. Incentives: To promote participation, the government offers incentives such as subsidies for screenings and medications for chronic conditions.
  5. Community Engagement: Residents can join health-related activities like group exercise classes or cooking workshops for healthier diets.

Singapore Citizens may enjoy fully-subsidised nationally-recommended vaccinations and screenings (Screen For Life) at your enrolled clinic.

For residents with chronic conditions

Such as diabetes, pre-diabetes, hypertension, hyperlipidemia, *gout, *chronic kidney disease, *heart disease. *from 1 Jan 2025

Enjoy subsidies of up to 87.5% for a selected whitelist of chronic medications for CHAS cardholders enrolled in our clinic. These medications would be similar to those prescribed at polyclinics and will cost exactly the same as polyclinic. Meaning you get to enjoy subsidised chronic medications at our clinic.

In addition, enjoy subsidies of up to $360 per year for other components of your chronic care under the Healthier SG Chronic Tier, such as doctor consultation and lab tests.

Our clinic is MediSave-accredited so you may use MediSave to pay for treatment of chronic conditions without any cash co-payment under the Chronic Disease Management Programme (CDMP).
